A. Responsibilities and Duties: The primary responsibility of the Supportive Housing Case Manager (CPST) is to work with people (mainly adults) whom are currently struggling with symptoms of mental illness, that also need assistance with obtaining and maintaining independent community housing. The consumers are served by the Bert Nash Community Mental Health Center.

1. To help clients to understand the symptoms of their mental illness and to minimize the effect of those symptoms in all the significant areas of their lives;

2. Complete strength assessments, collaborate in treatment planning and review the treatment plan with the client;

3. Assist clients in learning and practicing news skills that contribute to their recovery as specified in their individualized treatment plan;

4. Provide supportive counseling, solutions focused interventions; behavioral analysis to increase client's ability to function in all life domains;

5. Complete individualized assessments with clients and identify strategies or treatment options associated with the consumer's mental illness with the goal of minimizing the negative effects of mental illness symptoms or associated environmental stressors;

6. Providing outreach and crisis intervention services to clients and prospective clients to enable timely access to those services appropriate and necessary for crisis stabilization with the purpose of diverting the client from hospitalization and allowing the client to maintain independent living;

7. Coordinate the client's services with other providers, family, and community partners;

8. Delivering reimbursable hours of service compliant with established BNC standards;

9. Completing all paperwork associated with the financial, clinical, legal, or regulatory management of the case or BNC's operations in a timely and accurate manner;

10. Participating in public information and public relations activities, serve as a community liaison and advocate for mental health and the Bert Nash Community Mental Health Center;

11. Delivering reimbursable hours of service compliant with established BNC standards;

12. Additional responsibilities that may be assigned as circumstances warrant;

13. Flexibility to provide some after hour assistance clients in accessing the different locations within the Bert Nash CMHC network of services;

14. Regular and predictable attendance is an essential function of the job.


B. Accountability: The Supportive Housing Case Manager will be directly accountable to the Supportive Housing Program Supervisor (QMHP). The Supportive Housing QMHP or his/her designee will, no less than annually, complete a performance evaluation in compliance with personnel policies.

C. Qualifications: The Supportive Housing Case Manager position requires:

1. BA/BS degree (psychology, social work, or psychiatric nursing preferred) and/or four years of equivalent experience working in the human services field;

2. Ability to relate and effectively provide services to people who are experiencing mental health and housing related crisis;

3. Experience working with individuals who are experiencing a serious and persistent mental illness, have a history of substance use and might have experienced significant trauma;

4. Prior computer experience and basic computer fluency;

5. Must be able to achieve within one's first 6 months of employment a working competency in the management and use of the Bert Nash Community Mental Health Center's electronic medical record (EMR) software as demonstrated by timely and accurate documentation of the clinical record;

6. Must be able to lift 25 lbs or more to assist the client in ADLs, IADLs;

7. Must have reliable transportation and valid, unrestricted driver's license;

8. Strong oral and written communication skills;

9. An ability to work independently and in cooperation with others;

10. Strong organizational and time management skills;

11. Strong interpersonal skills in working with diverse populations;

12. Commitment in providing services with a Trauma Informed and Person-Centered approach;

13. Also brings an energetic and positive approach to the rapidly evolving changes within the housing program and the challenges of a complex workplace;

14. Flexibility in assigned working hours.
