Outreach Case Manager

Outreach Care Manager Outreach Care Manager responsibilities include engaging individuals identified by the Schenectady County Outreach HUB, assessing eligibility and needs based on individual goals, completing referrals to community resources, and ongoing assessment to work toward resolution of long-standing issues affecting care delivery, community stability, and health promotion. Attend Schenectady County Outreach HUB Boots on the Ground Meetings as scheduled and communicate with other HUB members on an ongoing and as-needed basis. Provide weekly intensive case management services to individuals until they are connected to permanent Health Home Care Management agencies. Provide comprehensive transitional care from inpatient to other settings to ensure individuals are linked with necessary services upon discharge or transition to another service provider. Regularly communicate with other care providers to coordinate information exchange, identify care requirements and needs, monitor service delivery and ongoing engagement, and advocate for additional services as needed. Provide transportation to clients using the agency’s vehicles. Promote a culture of inclusion and belonging. Skills/Qualifications A minimum of one year experience in case management, including assessment of needs, care planning, implementation and regular review, along with benefit entitlements, and linkage to community providers and services. Ability to function as a member of an interdisciplinary team as it relates to the mental health system of care. Knowledge of mental illness, substance abuse, physical/medical health, and homelessness. Valid and insurable driver\'s license. Agency vehicles are available for transporting clients. Must have a personal vehicle to use for business purposes when not transporting clients. Salary and Schedule $22.83/hour; $47,486/annual equivalent. 40 hours per week. Schedule can be flexible with 5 days at 8 hours/day from 9am to 5pm, with the following requirements: must work one weekend day, Tuesdays, and Wednesdays.
