Role Overview Admission RN assesses patients and initiates care into the services of Chapters Health System, ensuring a seamless transition to Care Navigation and acting as an ambassador. Location & Schedule 10a.m. – 7p.m., rotating weekends, DC and MD (Montgomery, Howard, Prince George’s). Benefits start on Day 1. Qualifications Current RN license in the state where the employee will be working. Minimum of one year nursing experience; hospice or hospital experience preferred. Previous experience with an EMR/EHR system. Strong clinical assessment skills; preferred background in emergency department or ICU. Ability to effectively communicate in English; bilingual in Spanish preferred. Ability to present services in a professional and articulate manner. Mobile Driver: valid driver’s license, automobile insurance, and reliable transportation. Ability to work in a team and manage multiple priorities, handling emotional stress. Ability to perform essential functions and physical requirements such as lifting patients/equipment, bending, pushing/pulling, kneeling, as accommodated. Strong goal/outcome orientation with exceptional time management. Excellent customer service focus. Active BLS certification from the American Heart Association or Red Cross. Competencies Successfully complete the competency requirements for this position. Core Responsibilities Complete clinical admissions for all Company services: explain services to patients/families, address questions and relieve fears, and present services compassionately. Identify admission objections, address concerns, and educate families on admission requirements. Complete informed consents and the initial assessment according to regulations and Company policy. Provide information to physicians and initiate the plan of care to address patients’ immediate needs. Initiate skilled nursing interventions to prevent complications and maximize comfort. Obtain physician orders and complete documentation per Company policy. Act as the Company representative at assigned facilities while facilitating referrals across all service lines. Communicate with Care Navigation staff to ensure timely follow‑up and inform them upon assignment completion. Provide necessary clinical communication using SBAR and discuss potential needs with after‑hours staff. Arrange safe discharge for referred and eligible patients, meeting productivity expectations. Obtain medical record information for the referral and develop strong relationships with case managers, physicians, and other facility staff. Work with Professional Relations Representatives to educate on available services. Provide care for GIP patients in hospitals, including daily visits, professional management, documentation, and efficient discharge. During emergencies (e.g., hurricanes) assist with triage, report to designated locations, and may be required to report early and stay overnight as needed. Perform other duties as assigned. Physical Demands Required abilities include: see, hear, talk, walk, use hands to finger, handle or feel; frequently stand, sit, reach with hands or arms, lift, bend, balance; occasionally pull, push, stoop/crouch, kneel, climb stairs. Compensation Pay range: $80,095.29–$119,213.84. Drug and alcohol testing is required after a conditional offer and ongoing compliance with the Drug‑Free Workplace Policy.
