Admission Nurse

Admission Nurse (RN) – Home Health Prince George’s County & Montgomery County, Maryland Human Touch Home Health is seeking an experienced and dependable Admission Nurse (RN) to provide home health admission and Start of Care visits throughout Prince George’s County and Montgomery County, Maryland . We are looking for an RN who is confident working independently in the field and understands that a thorough admission sets the foundation for quality patient care. Key Responsibilities Complete Start of Care (SOC) and admission visits in patients’ homes Perform comprehensive nursing assessments Complete OASIS documentation accurately and within required timeframes Conduct medication reconciliation and identify potential concerns Assess patients’ clinical, safety, environmental, and support needs Develop and initiate individualized plans of care Educate patients and caregivers regarding diagnoses, medications, treatments, and the home health plan of care Coordinate care with physicians and members of the interdisciplinary team Communicate changes in patient condition or concerns promptly Complete required documentation accurately and timely Maintain compliance with Medicare Conditions of Participation, agency policies, and applicable regulations Qualifications Current and unrestricted Maryland Registered Nurse (RN) license Minimum of one year of professional nursing experience preferred Home health experience strongly preferred OASIS and Start of Care experience strongly preferred Strong clinical assessment and documentation skills Excellent communication, organization, and time-management skills Ability to work independently in a field-based position Reliable transportation and valid driver’s license Ability to travel throughout Prince George’s County and Montgomery County Service Area This position provides home health admission coverage throughout: Prince George’s County, MD Montgomery County, MD If you are an experienced RN who understands Start of Care, OASIS, and home health admissions , we want to hear from you.
