Essential Duties and Responsibilities include the following. Other duties may be assigned.

• Completes paper medical records chart reviews and assists with maintenance, purging and storage of records according to regulation. 
• Review EHR admissions for duplicate encounters and assist with removing them or notifying proper personnel to prevent errors.
• Provides general office/phone coverage in a professional manner.
• Is trained on and understands the flow of the medical record through the facility from admission to discharge, and assists with effective record flow and processing.
• Navigates the filing/retrieval system of the paper medical records, and is able to locate a paper chart and obtain needed information as necessary to process medical records requests and inquiries.
• Efficiently uses the Electronic Health Record (EHR) i.e. is able to effectively locate, compile and complete records as requested. Is able to locate needed information that has been scanned, documented or entered into the EHR.  Can proficiently assist others with locating or providing such information.
• Comprehends basic release of information guidelines and performs such tasks accurately and in compliance with HIPAA and confidentiality standards.
• Adheres to confidentiality and security of all forms of patient information. 
• Completes medical records requests timely and accurately, following all HIPAA compliance and state/federal requirements as well as internal policies. Completes medical records requests for all GCHS locations (hospital, Greeley & Wallace County Clinics, etc.).
