Exciting Opportunity for a Social Worker in Acute Care! Job Description:
- Start Date: ASAP
- Shift: Day 5x8-Hour (08:00 - 16:30)
- Location: Laguna Facility
- Patient Ratios: Caseload of 20-23 patients
- Experience Required: Minimum of 1 full year of acute care experience
- Weekend Requirement: Variable 5-day workweek with department operations occurring 7 days per week. Days off may vary and are not guaranteed to be back-to-back. Key Qualifications:
- State License: California state license required (no RTO restrictions)
- Travel Experience: Required
- Certifications: BLS, LCSW*, MSW
- Minimum Years of Experience: 1 year of acute care hospital experience required
- Other Experience: Trauma Level I & II experience preferred, community hospital experience a plus Skills Required:
- Admission Criteria
- Care coordination
- Case management
- Discharge Planning
- Biopsychosocial assessment
- Documentation according to plan of care
- Knowledge of various health programs and services Position Guidelines:
- Schedule: Primarily Sunday through Thursday; subject to change based on staffing needs
- Patient Age Groups: Adults, Geriatrics
- Daily Census: 25 patients
- Team Collaboration: Work closely with RN's and other multidisciplinary team members to coordinate the discharge plan
- Unit Details: Responsible for initial assessments within 24 hours, ensuring a seamless transition for patients to post-acute care Additional Information:
- Floating: Travelers may be required to float to nearby sister facilities
- Orientation Hours: 40 hours
- References Required: Two references covering 1 year of experience
