Nurse Practitioner – Fee-for-Service

Home-Based Geriatric Primary & Transitional Care

Care Concierge Solutions is seeking a compassionate and experienced Nurse Practitioner (NP) to provide home-based primary and transitional care to geriatric patients throughout the community.

This is a fee-for-service position offering flexibility and the opportunity to see patients in their homes while providing personalized, high-quality medical care.

Position Responsibilities

• Provide in-home primary care to geriatric and medically complex patients
• Perform comprehensive new patient evaluations and follow-up visits
• Provide Transitional Care Management (TCM) visits following hospital and skilled nursing facility discharges
• Evaluate and manage acute and chronic medical conditions
• Perform medication reconciliation and management
• Review hospital records, laboratory results, imaging, and other diagnostic information
• Develop individualized treatment and care plans
• Coordinate care with specialists, hospitals, home health agencies, facilities, caregivers, and family members
• Communicate clearly with patients and families regarding diagnoses, treatment options, and care plans
• Recognize changes in patient condition and determine appropriate treatment or escalation of care
• Complete accurate and timely documentation in the electronic medical record
• Participate in care coordination designed to improve outcomes and reduce avoidable hospitalizations

Compensation & Schedule

• Fee-for-service compensation based on completed patient visits
• Flexible scheduling based on patient volume and NP availability

• Visits are primarily conducted in patients' homes

Qualifications

• Current, unrestricted autonomous Florida Nurse Practitioner license
• National NP board certification
• DEA license
• Medicare enrollment or eligibility to enroll
• Strong clinical assessment and decision-making skills
• Excellent communication and interpersonal skills
• Ability to work independently in a home-based environment
• Reliable transportation
• Strong organizational skills and attention to detail
• Commitment to timely and thorough clinical documentation

Geriatric, primary care, home health, hospital, SNF, or transitional care experience is strongly preferred.
