Position Overview

We are seeking a qualified candidate with experience in acute hospital case management to join our team. This role involves monitoring patient care and discharge planning, as well as coordinating services for a diverse patient population. The ideal candidate will demonstrate a strong commitment to quality patient care and possess essential skills in social work and case management.

• Shift: Day 5x8-Hour (08:00 - 16:30)
• Start Date: ASAP
• Years of Experience Required: 2 years of acute hospital case management experience
• Weekend Requirements: Every other weekend
• Holiday Coverage: Must be available for holiday coverage

Job Responsibilities

• Monitor and manage a caseload of up to 25 patients.
• Focus on discharge planning and coordination of care services.
• Collaborate with multidisciplinary teams to ensure continuity of care.
• Provide patient and caregiver education to facilitate successful discharges.
• Participate in family meetings and care conference activities.
• Document patient assessments, care plans, and interventions.
• Assist patients in navigating services such as home healthcare and DME needs.

Qualifications

• Required: 2 years of acute hospital case management experience.
• Experience with discharge planning strategies.
• Knowledge of community resources and support services.
• Experience with electronic health record systems, particularly Epic.
• CA state license preferred; pending licenses accepted.
• Certifications in relevant areas of social work and case management are beneficial.

Skills Required

• Advanced knowledge of case management principles and practices.
• Strong communication and interpersonal skills.
• Ability to assess and respond to patient and family needs effectively.
• Understanding of legal issues in healthcare, including advanced directives and guardianship.
• Experience with mental health assessments and working with vulnerable populations.

Additional Information

This position requires flexibility to adapt to the needs of the facility and the patients. The candidate may be required to float to different units as needed. Participation in ongoing training and education regarding case management is essential for success in this role.
