Clinical Records Clerk

Position Summary Appropriately and accurately verify charts within the Electronic Health Record (EHR). Assist staff to maintain accurate documentation by assigning document requirements within the EHR. Accurately scan and index documentation to the appropriate chart. Release information to patients, healthcare facilities and other entities while following all laws and regulations, including HIPAA. Essential Duties And Responsibilities Lives out the INcompass Healthcare promise by consistently displaying compassion, treating everyone with dignity, and providing exceptional care. Incorporates the six principles of trauma‑informed care throughout all aspects of their work. Observes confidentiality and safeguards all patient‑related information. Maintains a good working relationship within the department and with other departments. Assists clinical staff with completion of documentation in the EHR. Answers the phone to assist staff with documentation requirements and other questions as they arise. Appropriately scans and indexes paper documents into the EHR in a timely manner. Assigns document requirements to charts (e.g., treatment plans due dates, ANSA/CANS due dates, etc.). Assists as needed to verify accurate information submitted to DARMHA. Reviews release of information for appropriate completeness, coordinating the release of records to other agencies, lawyers, insurance companies, state and federal agencies. Assists in data collection activities as they arise.
