Job Description:

Reports to: Resident Care Coordinator

JOB SUMMARY:
The Resident Outings Coordinator is primarily responsible for scheduling resident medical appointments and coordinating transportation to and from those appointments in a timely, organized, and resident-centered manner. This includes managing transportation logistics, maintaining communication with healthcare providers and family members, and ensuring residents are safely and appropriately supported before, during, and after outings. This position also provides support to the Resident Care Coordinator (RCC) with administrative, documentation, and care coordination responsibilities as needed.

PRIMARY RESPONSIBILITIES:

Appointment Scheduling & Transportation Coordination:

• Schedule and confirm medical appointments for residents, including primary care, specialists, therapy, dental, and other services as ordered.
• Coordinate transportation services for resident appointments, including contracted providers or family escorts.
• Ensure all necessary documentation (face sheets, medication lists, insurance info) accompanies the resident to the appointment.
• Maintain accurate records of appointments, transportation requests, and resident attendance.
• Follow up on missed or rescheduled appointments and communicate updates to appropriate staff.

Resident Support & Safety:

• Confirm residents are prepared for outings, including mobility equipment, personal items, and appropriate supervision if needed.
• Ensure timely resident pick-up and drop-off, communicating delays or concerns with staff and family as appropriate.
• Collaborate with care staff and RCC to ensure each resident's needs are met while away from the community.
• Ensure Residents are accompanied and supported by preferred staff on outings.
• Report any incidents or changes in condition observed during transport or upon return.

RCC Support Responsibilities:

• Assist the Resident Care Coordinator with:
  • Filing and organizing medical paperwork.
  • Assembling new admission charts and dismantling discharge files.
  • Tracking and documenting appointment outcomes and follow-up orders.
  • Inventorying and ordering care supplies, if assigned.
• Help facilitate communication between families, providers, and the care team regarding appointment logistics or medical updates.
• Participate in quality assurance audits, shift reports, and other health services processes as directed.

General Responsibilities:

• Maintain confidentiality and comply with HIPAA and company policies at all times.
• Attend staff meetings and training sessions as required.
• Perform other duties as assigned by the RCC, Director of Nursing Services, or Executive Director.

QUALIFICATIONS:

• Genuine concern for and ability to work with the elderly.
• Knowledge of the requirements for providing care and supervision to the elderly.
• Ability to delegate responsibility while maintaining focus of daily activities.
• Ability to manage and prioritize a complex medication and treatment administration process, within a specified timeframe.
• Must be able to utilize standard precaution knowledge and infection control measures where required.
• Organized and knowledgeable of basic medical record filing styles.
• Possess sufficient communication and language (orally and written) skills to perform job duties, interpret medical information and communicate with residents, other staff, family members, etc., as needed.
• Must be 18 years of age or older.

EDUCATION:

• Minimum high school graduate or GED.
• Preferred coursework in medical terminology, pharmacology, or basic health services.
• CPR/first aid certification required.

EXPERIENCE:

• Minimum six months experience as a Med Aide, prefer previous experience as a Lead Med Aide.

PHYSICAL JOB REQUIREMENTS:

In a typical eight hour day, employee will:

• Stand/walk frequently, punctuated by opportunities to sit.
• Sit frequently, up to 3-4 hours at a time, punctuated by varying intervals to stand.
• Constantly | Frequent | Occasional | Rarely.

• Bend at waist, knees, hips and spine, frequently while lifting ≥10# but ≤30#.
• Secure proper assistance for transferring and lifting of residents as needed (training required for this task).
• Perform squats on a frequent basis to reach below knee requiring ≥10# but ≤30#.
• Reach on a constant basis while performing filing functions. May occasionally reach overhead requiring ≤10#, and below waist requiring ≤30#.
• Perform a twist or rotation frequently at the trunk, hips, knees and neck while performing duties requiring ≥10# but ≤30#.
• Vision-must be able to read clearly with or without corrective lenses.
• Hearing- Must be able to hear telephone, audible alarms, bells, and signals related to resident safety, with or without hearing devices.
• Speech/language- must have strong command of the English language sufficient to read and write and interpret medical information.
• Bloodborne exposure category: High-Offer vaccine. Exposure tasks include: Injections, direct resident care.
