Advanced Practice Provider Palliative Care

Advanced Practice Provider – Palliative Care Position: full-time day shift. The Advanced Practice Provider is accountable for advanced nursing practice in the care of assigned patient populations. Responsibilities Collect and analyze client health data. Identify appropriate expected outcomes and develop and implement a plan of care. Provide comprehensive clinical coordination of care and case management. Provide consultation to influence plan of care for clients. Enhance the abilities of others and effect change in the system. Use prescriptive authority, procedures, and treatments in accordance with state and federal laws and regulations. Evaluate client progress in attaining expected outcomes. Conduct nursing research. Provide education to patients, families, and nursing staff. Case manage high-risk patients. Schedule Weekend, on‑call, and holiday commitment required. Location Saint Luke's Plaza, with expectation of floating to additional sites at intervals. Requirements and Experience Applicable experience: less than 1 year Advanced Practice Provider (MO), Advanced Practice Provider Cert (MO/KS), or Advanced Practice Provider – various.
