Join a dedicated healthcare team as a Home Health Speech Therapist (SLP), providing compassionate, patient-centered care in the comfort of patients' homes. In this role, you will evaluate, diagnose, and treat communication, cognitive, swallowing, and speech disorders while developing individualized treatment plans that promote independence and improve quality of life. Working collaboratively with physicians, nurses, therapists, patients, and caregivers, you will deliver skilled therapy services, monitor patient progress, and provide education to support successful outcomes. This position offers the opportunity to make a meaningful impact through personalized care while enjoying a high level of autonomy and flexibility in a home health setting.

Experience and Education

Required Education: Undergraduate degree

Experience: 1 year to two (2) years in Speech Therapy

Licenses and Certifications:

Basic Life Support course accredited by the American Heart Association (AHA) or American Red Cross

Valid Texas driver's license, proof of driver's insurance

Current licensure as a Speech Therapist

Job Duties

Evaluates patient's home environment and communicative status of patient.

Develops plan of care in coordination with physician (when required).

Provides patient care and coordinates the care with other disciplines and team members.

Coordinates patient care with other disciplines and team members.

Documents care and interventions.

Participates in the implementation of the Quality Improvement Plan.

Maintains a safe and healthy environment.
