Remote Telephonic Care Manager - LTSS Waiver Specialist
Molina Healthcare of Illinois
Molina Healthcare in Texas is seeking an experienced Case Manager for a fully telephonic role, residing in Texas. The position requires 2+ years of care management, with LVN, LBSW, or LMSW preferred but not required if experience meets the threshold. Monday to Friday, 8 AM–5 PM CST, with 25–40% travel as state contracts allow. You will conduct comprehensive assessments, develop care plans, coordinate LTSS and behavioral health services, and collaborate with a multidisciplinary team to deliver
