Remote LTSS Care Manager: Waiver & Care Coordination

Molina Talent Acquisition

Molina Healthcare in Wisconsin is seeking a care management professional to support long-term services and supports (LTSS) coordination for high-need members. The role requires collaboration with a multidisciplinary team and may include in-person home visits, with 25–40% local travel. You will assess needs, develop and implement care plans, monitor progress, coordinate waiver enrollment/disenrollment, and promote integration of behavioral health, LTSS, and community resources to improve outcomes
