JOB TITLE: Home Health PDN (Private Duty Nurse) - LPN or RNActive Ohio LPN or RN in good standing is required.At least 1 year of paid experience at a home health agency (or equivalent) is required; 2-5 years' experience is preferred.Minimum of 1 year of experience is required in: trach care including suctioning, vent care, PEG tube feedings and medication administration, respiratory treatments (incl. nebulizer, shake vest, cough assist), wound care, colostomy care, catheter care, Hoyer lift use and transfers, and ROM exercises. 1+ years of experience in the following is desirable but not required: Case Management, PICC/PORT/IV Care, lab draws, and OASIS and Non-OASIS assessments. SCHEDULE TYPE & SERVICE AREA: Oak Harbor, OH (43449 | Ottawa County)Immediate starting commitment of 30 hours/week is required, with flexibility on exact days and times, primarily focused on weekday / daytime coverage and occasional weekends. Additional client(s) will be sought if 31-40 hours per week are desired (not required).Ability to consistently, reliably provide services in Oak Harbor, OH is required for Part-Time (30hr/wk) employment, or Oak Harbor Plus up to a 30-minute radius from your home is required. for Full-Time employment.Qualified applicants may reside in or near any primary service area so long as travel requirements are fulfilled throughout the assigned region.EMPLOYMENT TYPE: Long-term employment for 30 hrs/wk (PT) or up to 40 hrs/wk (FT)No On-Call shifts required. *This is a long-term employment position requiring a recurring weekly schedule. Additional Home Health Nursing experience/ familiarity is highly desired.Active CPR and First Aid certificates highly desired (for speed to start care), but agency will provide recertification if required.Minimum of 1 year direct patient experience with vent care is required.Good interpersonal communication skills - written and verbal.Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful.Current driver's license or state ID, plus reliable transportation, are required.Excellent attendance record with past employers.Ability to meet all of the basic job requirements is a must.Willingness and ability to pass state and federal background check. *Compensation is based on employee qualifications and certifications, experience, skill specializations, positive employment history, and overall client & team fit. Position SummaryA successful PDN at Home Care Network, Inc., performs skilled nursing functions in accordance with the federal, state, and local laws and within the guidelines of his/her professional organization and agency. All functions shall be performed in accordance with the established policies and practices and state Nurse Practice Act. The LPN/RN delivers care to clients as delegated by the RN Case Manager.Reports to: RN Case Manager; Clinical Administrator/Director of Nursing Essential Functions/Areas of Accountability:Provides professional nursing care under the direction of a Registered Nurse. Communicates appropriate status/condition changes to the RN per agency guidelines.Provides skilled nursing visits for clients as directed by the RN Case Manager. Follows the care plan established by the Registered Nurse.Recognizes and reports changes in client condition to the Case Manager and physician, as directed.Communicates information on a timely basis, consistent with agency policy and urgency of the situation.
