This position requires flexibility, attention to detail, strong problem-solving skills, and a commitment to delivering an exceptional patient experience The RN will support care team decision-making, participate in weekly care team meetings, provide patient education, coordinate clinical and auxiliary services, and help ensure patients receive high-quality, prevention-focused care The role also includes telephone support and triage, symptom management, care coordination, medication and testing authorizations, and collaboration with Medical Assistants and other members of the care team The RN may periodically perform home visits and will be expected to demonstrate positive energy, accountability, clinical excellence, and a strong commitment to patient-centered care Job Duties Provide telephone support and triage for patients Schedule problem visits or nurse visits when appropriate Offer symptom management and support refill requests Answer patient visit follow-up questions Provide patient education, particularly regarding chronic conditions Provide education in person, over the phone, or in groups Monitor and coordinate delivery of patient care plans Build relationships with patients and caregivers Support prevention-focused care and ED/hospital diversion where appropriate Provide direction to Medical Assistants to support procedures, telephonic support, and patient instructions Conduct medication and testing authorizations Deliver an exceptional patient experience through service, responsiveness, and care Participate in weekly care team meetings Coordinate clinical and auxiliary services needed to provide high-quality patient care May periodically perform home visits Perform other duties as assigned Travel May periodically perform home visits #CV1 **Only those lawfully authorized to work in the designated country associated with the position will be considered.** **Please note that all Position start dates and duration are estimates and may be reduced or lengthened based upon a client’s business needs and requirements.** Benefits For information and details on employment benefits offered with this position, please visit here.
