This full-time position offers a competitive salary range of USD 87,000 – 105,000 annually, commensurate with experience and qualifications.

Key Responsibilities\n

• Conduct comprehensive patient intakes that focus on diagnostic and functional evaluations while making informed recommendations to the primary care team regarding treatment goals and plans.

• Provide focused therapeutic interventions for patients in need of mental health support, ensuring adherence to established guidelines set by the Agency and Federally Qualified Health Center (FQHC).
\n
• Collaborate with primary care providers to assess and manage mental health and psychosocial issues effectively, contributing as an integral member of the primary care team.
\n
• Monitor and document patient progress, ensuring all relevant information is captured in treatment charts while keeping primary care providers informed of patients’ needs.
\n
• Develop and implement relapse prevention plans, initiating follow-up with patients to evaluate their progress and adjust treatment strategies accordingly.
\n
• Assist in identifying at-risk patients, coordinating care efforts with other providers while respecting patient confidentiality.
\n
• Educate patients on self-management skills and lifestyle changes to promote healthier behaviors and recovery.
\n
• While working closely with clinic management, provide insights on behavioral services, suggesting areas for program evaluation and improving patient outcomes.

Qualifications\n

• A minimum of a Master’s degree in Clinical Counseling from an accredited institution.

• Current LPCC license is required for practice.

• Ability to maintain thorough clinical records and documentation in compliance with health regulations.

• Excellent communication skills, with the capability to establish and nurture effective relationships with patients, families, and healthcare professionals.
