Weekly Hour Guarantee: 32-40 hours Productivity Expectations: 30 points Coverage Areas: York County & Western Adams County Schedule: Monday-Friday, 8:00 AM-4:30 PM On-Call / Weekend Requirements: None Mileage Reimbursement: Yes - reimbursed between visits, from first patient to last patient Pay Rate: $49/hr


Position Overview


The Physical Therapist will serve as the primary PT for the agency , providing comprehensive home-based physical therapy services. This role requires strong clinical judgment, autonomy, and excellent communication skills to manage a full caseload and ensure high-quality patient care.


Top Responsibilities

• Start of Care Assessments - Complete SOCs, evaluations, reassessments, and discharges
• Plan of Care Development - Establish individualized POCs aligned with patient needs and goals
• Caseload Management - Independently manage and prioritize daily patient visits
• PTA Supervision - Provide oversight and guidance to Physical Therapist Assistants
• Patient Scheduling - Call and schedule patients daily to maintain continuity of care
• Documentation - Complete accurate, timely documentation in accordance with agency and regulatory standards

Required Qualifications

• PT degree from an accredited program
• Active state licensure
• Home health experience preferred
• Valid Physical Therapist (PT) degree and licensure
