Role Overview The Care Coordinator supports children with developmental, physical, or emotional disabilities through assessment, service planning, and coordination within the Children's Long Term Support (CLTS) Waiver Program. They serve as the primary contact for families, ensuring access to services that promote safety and well-being. Key Responsibilities Conduct assessments and develop individualized service plans with families and providers Authorize and coordinate services to meet the plan's goals Facilitate family-centered team meetings and maintain accurate documentation Collaborate with service teams and, if needed, testify at legal proceedings Participate in staff training and development Direct Service Expectations Promote client independence and growth Provide trauma-informed, family-centered services using evidence-based practices Accurately complete time reporting Qualifications & Skills Bachelor’s degree in a health or human services field At least 1 year of experience working with children in the target population Strong communication, organization, and documentation skills Proficiency with electronic health records and computer applications Valid driver’s license and reliable transportation Bilingual Spanish skills are preferred but not required #J-18808-Ljbffr Alpha Home Health and Hospice
