Care Coordinator

Role Care Coordinator for a Birth to Three Program, supporting children aged 0-3 with developmental delays or disabilities and their families. Responsibilities Partner with families, service providers, and community resources conduct assessments develop and implement individualized, family-centered service plans coordinate and facilitate services provide education and advocacy maintain accurate documentation lead family team meetings collaborate with internal teams and community partners manage flexible schedules meet productivity goals. Qualifications Bachelor's degree in Social Work, Child Development, or related field preferred experience working with families with special needs valid driver's license and reliable transportation strong organizational, communication, and documentation skills ability to work in home, office, and community settings respond effectively in crises. Work Environment Community-based with travel; includes physical activity and occasional overnight stays; serving families in diverse environments.
