We are seeking a compassionate and skilled Home Health Registered Nurse RN to join our team of dedicated healthcare professionals. Responsibilities Provide patient‑centered care to patients in their homes, focusing on their unique needs and goals Conduct initial assessments, develop and implement individualized care plans, and evaluate patient progress Collaborate with interdisciplinary teams, including physicians, therapists, and caregivers, to ensure comprehensive care Administer medications, treatments, and therapies as prescribed by physicians and in accordance with agency policies Document patient information accurately and in a timely manner, ensuring compliance with regulatory requirements Participate in quality improvement initiatives to enhance patient outcomes and agency performance Requirements Current RN licensure in the state of California Minimum 1 year of experience in home health or a related field Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and interdisciplinary teams Ability to lift, move, and transfer patients as needed Reliable transportation and the ability to travel to patients' homes #J-18808-Ljbffr Pathwellhealth
