Care Manager

Northshore Home Health Care

Role Overview The caregiver provides hands-on support to residents, promoting their safety, dignity, and independence. Responsibilities include assisting with activities of daily living (ADLs) such as bathing, dressing, grooming, mobility, and personal care, and supporting dining services. They follow Individualized Service Plans (ISP) , document changes, and create engaging experiences, especially for residents with memory loss in Memory Care. Effective communication with families and care team members is essential, along with maintaining a clean, welcoming environment. Qualifications & Skill High School Diploma or GED, age 18+, CPR/First Aid certification (or ability to obtain), and qualities like compassion, reliability, and strong communication skills.
