Behavioral Health Specialist

Oak Street Health

Oak Street Health seeks a Behavioral Health Specialist to provide integrated mental health services for adult Medicare patients in underserved communities. In this role, you will deliver assessments, brief therapy, and care planning for patients with depression, anxiety, and other behavioral health needs, collaborating closely with primary care providers and a multidisciplinary team. You will use evidence-based interventions, track outcomes, coordinate referrals, and support crisis intervention as needed. Oak Street Health offers a patient-centered, inclusive culture with strong support for professional growth and impactful community-focused work.
