Responsibilities
- Perform patient evaluations and develop individualized physical therapy treatment plans for geriatric residents.

- Collaborate with the interdisciplinary care team to coordinate care and support discharge planning.
- Educate residents and families on therapy goals, safety, and activity programs; promote function and quality of life.
- Ensure compliance with facility policies, accreditation standards, and applicable regulations.

Requirements
- Active Physical Therapist license in good standing or eligible for licensure in West Virginia.
- Ability to work effectively in a skilled nursing facility environment.
- Strong communication, documentation, and collaboration skills.
- Commitment to patient-centered care, safety, and quality outcomes.
- Availability to meet guaranteed hours (35 per week) with adherence to the scheduled shift.
