LTSS Service Coordinator - Clinician (LSW, LCSW, LPN)

LTSS Service Coordinator – Clinician (LSW, LCSW, LPN) Location : Field-based. Operate primarily in the field, traveling to client sites or designated locations as required, with occasional office attendance for meetings or training. Alternate locations may be considered if candidates reside within a reasonable commuting distance from an office. Candidates not within a reasonable commuting distance from the posting location(s) will not be considered for employment unless an accommodation is granted as required by law. Responsibilities Assist in identifying members for high risk complications. Obtain clinical data as directed. Identify members who may benefit from an alternative level of care or other waiver programs. Provide all collected information to the responsible RN, who verifies and interprets the information, conducts additional assessments as necessary, and develops, monitors, evaluates, and revises the member’s care plan to meet the member’s needs. Participate in coordinating care for members with chronic illnesses, co‑morbidities, and/or disabilities in conjunction with the RN, the member, and the health care team to ensure cost‑effective and efficient utilization of health benefits. Make decisions based on the current needs of the member, understanding of disease processes and terminology, and application of clinical guidelines; nursing judgment is not required. The LTSS Service Coordinator‑Clinician contributes to the LTSS care coordination process by performing activities within the scope of licensure, including assisting the responsible RN with telephonic or face‑to‑face assessments for identification, evaluation, coordination, and management of member’s needs, such as physical health, behavioral health, social services, and long‑term services and supports. Minimum Requirements Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or other license (not RN) as allowed by state law. Nursing Diploma or AS in Nursing or related field. Minimum of 2 years of experience working with individuals with chronic illnesses, co‑morbidities, and/or disabilities in a Service Coordinator or similar role, or an equivalent combination of education and experience. License must be valid in the applicable state(s). Preferred Skills, Capabilities and Experiences Strong preference for case management experience with older adults or individuals with disabilities. BA/BS in Health or Nursing preferred.
