We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Holmes Lake Rehabilitation & Care Center in Lincoln, Nebraska. About the Role:

As a Licensed Practical Nurse (LPN) at Holmes Lake Rehabilitation & Care Center, you will be responsible for providing direct resident care to our residents. This includes administering medications, performing vital signs, and completing wound care. You will also be responsible for documenting resident information and communicating with other healthcare providers.

Responsibilities:

• Provide direct patient care to residents, including administering medications, performing vital signs, and completing wound care
• Document resident information and maintain accurate records
• Communicate with other healthcare providers to ensure seamless care coordination
• Participate in interdisciplinary team meetings to develop and implement resident care plans
• Provide emotional support and education to residents and their families
• Collaborate with other healthcare professionals to ensure high-quality care

Requirements:

• Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Nebraska
• Ability to work with a diverse population and demonstrate a commitment to customer service
• Strong communication and interpersonal skills
