Lakeland

1. A.S – L1

• Schedule: Sunday 7a–7p, Thursday 7p–7a, Friday 7a–7p
• Hours: 36 hrs/week
• Rate: $28 base / recommended $34.53
• Skills: HT – LVN

2. M. H. – 18 y/o L3

• Schedule: Mon–Fri 7a–7p
• Hours: 60 hrs/week
• Rate: $28 base / recommended $31.53
• Skills: LT – LVN
• Important: Male nurses only

3. R.S – 38 y/o

• Schedule: Tues–Thurs 3p–11p
• Hours: 24 hrs/week
• Rate: Recommended $31.03
• Notes: Must be comfortable wearing a mask around patient; 2 small dogs

4. CxM – Toddler

• Schedule: Mon–Fri, approximately 8a–3p
• Hours: 35 hrs/week
• Skills: G-tube, trach, vent
• Notes: Nurse attends school with patient; admission pending nurse coverage/school approval

5. GxP – Toddler

• Schedule: Thursday & Friday 8a–3p
• Hours: 14 hrs/week
• Rate: $28–30/hr
• Skills: G-tube, mild hearing loss, O2, non-invasive vent PRN
• Notes: Nurse accompanies PCG to appointments

6. KxM – Preschooler

• Schedule: Saturday 7a–7p + Saturday night 7p–7a; additional night nurse 2 shifts/week
• Hours: Approximately 48 hrs/week
• Rate: $32–34/hr
• Skills: G-tube, trach, vent
• Male or female nurses

North Lakeland

1. CxA – 2.5 y/o

• Schedule: Mon–Sun, 7p–7a
• Hours: 84 hrs/week available
• Rate: $28–30/hr
• Skills: Failure to thrive, G-tube
• Preference: Female nurses preferred

2. OxL – 3 y/o

• Schedule: Friday 7a–7p; Saturday & Sunday 9a–7p
• Hours: 32 hrs/week
• Rate: $27–29/hr
• Skills: Microcephaly, G-tube, diaper changes
• Requirement: Female nurses only
• PRN: Yes

Qualifications

• Must have and maintain an active, unencumbered license (LPN/LVN) in the state in which the clinician will practice
• Compact licenses must be transferred to your state of residence within 90 days   
• Current CPR certification (with hands-on component)- Aveanna can assist in obtaining this requirement after hire, if necessary.
• TB skin test (current within last 12 months)
• Six months prior hands-on nursing experience preferred but not required
• Must have reliable transportation
