Description Job Description: Care Coordinator Position Summary The Care Coordinator serves as a primary point of contact for individuals seeking services and support. This position is responsible for conducting client screenings and intakes, coordinating referrals, connecting clients to community resources, and ensuring timely follow-up throughout the service process. The Care Coordinator plays a vital role in helping clients navigate housing, employment, healthcare, and other support systems while maintaining accurate records and communication. Location: 95 W 11th St. Suite 206 Tracy, CA 95376 (in person) Department: Case Management Reports To: Director of Programs FLSA Status: Full-Time Non-Exempt Essential Duties and Responsibilities Client Intake and Assessment Conduct initial client screenings, assessments, and intake interviews. Welcome and assist walk-in clients, phone inquiries, and individuals seeking services. Gather and document client information accurately and confidentially. Document all client contact as encounters in In Sync. Assess client needs and determine eligibility for available programs and services. Assign clients to the appropriate Case Manager, program, or service provider based on identified needs. Care Coordination and Referrals Coordinate referrals to internal programs and external community partners. Track referrals and ensure timely follow-up with clients and service providers. Monitor client engagement and service progression to ensure continuity of care. Maintain communication with clients regarding appointments, referrals, and service updates. Assist clients in accessing housing, employment, healthcare, benefits, and other supportive services. Housing and Employment Resource Support Research, compile, and distribute current housing availability lists and employment opportunity listings. Create and send regular housing and job resource updates to clients. Maintain relationships with community partners and resource providers to ensure information remains current and accurate. Client Communication and Administrative Support Monitor and respond to the Pear Team email inbox in a timely and professional manner. Assist with incoming phone calls, client inquiries, and requests for information. Sort and distribute client mail and correspondence. Maintain accurate client records, case notes, and referral documentation. Ensure compliance with organizational policies, confidentiality requirements, and data collection procedures. Collaboration and Program Support Work closely with Case Managers, outreach staff, volunteers, and community partners to coordinate client services. Participate in team meetings and case consultations as needed. Assist with special projects, events, and program activities that support organizational goals. Provide compassionate, trauma‑informed, and client‑centered support to all individuals served. Requirements Qualifications Associate degree in Social Services, Human Services, Psychology, Public Health, or a related field preferred; equivalent experience may be considered. Experience working with vulnerable populations, community services, housing programs, or case management preferred. Strong organizational skills and attention to detail. Excellent verbal and written communication skills. Ability to manage multiple tasks and priorities in a fast‑paced environment. Proficiency with Microsoft Office, Google Workspace, databases, and email systems. Ability to maintain confidentiality and professional boundaries. Commitment to serving individuals with dignity, respect, and compassion. Work Environment: This position requires working in an office setting Interaction with clients experiencing housing instability and other challenges is frequent. Working Conditions None: The worker is not substantially exposed to adverse environmental conditions (as in typical office or administrative work). Physical Requirements Sedentary work : Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met. Talking : Expressing or exchanging ideas by means of the spoken word; those activities where detailed or important spoken instructions must be conveyed to other workers accurately, loudly, or quickly Hearing : Perceiving the nature of sounds at normal speaking levels with or without correction, and having the ability to receive detailed information through oral communication, and making fine discriminations in sound.
