CVHCare, a leader in Home Health Clinical services, headquartered in beautiful San Ramon, CA, is currently seeking a Physical Therapist to join our Home Health Care Agency located in Woodland Hills, CA. Schedule: Per Diem (6 visits per week) Locations Available: Area #40 covering the cities of North Hollywood, Sherman Oaks, Studio City, Sun Valley, Toluca Lake, Universal City, Valley Glen, Valley Village, Van Nuys and Area #44 covering the areas of La Canada, La Crescenta, Burbank, Montrose, Shadow Hills, Glendale, Sunland and Tujunga Paid Training: Must be able to train for 5 days Mon-Friday 9am-5:30pm in Woodland Hills

Position Summary: Initially, a PT will evaluate mobility, strength and current level of function, while setting goals to improve balance, walking, transfers, as well as patient education. Unlike a rehabilitation setting or even an outpatient clinic, unique emphasis is placed on safe mobility in and around the patient's residence. PT's will also make recommendations on equipment and provide train on proper use of equipment.

Requirements:

• 1 year Physical Therapy experience
• Open to entry level or candidates with no prior home health experience
• Reliable Transportation
• CPR/BLS Certification
• Must be available for one week of orientation and training
