Contract | Full-Time | Days (8:00 AM – 5:00 PM) | 40 Hours/Week | Weekend rotation every 4–6 weeks | Up to 26 weeks

Key Responsibilities

• Evaluate patient functional abilities and disabilities including range of motion, strength, and coordination through standardized assessment; establish functional baseline and monitor progress throughout treatment
• Interpret and implement referrals; develop and implement individualized education and treatment plans including goal setting and estimating length of treatment
• Provide direct patient care including individual and/or group physical therapy activities in accordance with the treatment plan across Van Buren, Calhoun, and Kalamazoo Counties
• Participate in interdisciplinary team discussions, rounds, and clinics; provide family and community education
• Maintain accurate, legible, and complete documentation in the electronic health record per compliance timelines, including evaluations, treatment plans, and progress notes
• May design and fabricate splints and assistive devices; participate in program development and department process improvement initiatives

Required Qualifications

• Active Michigan Physical Therapist license required, in good standing
• BLS certification through the American Heart Association (AHA) required
• Minimum 1 year of physical therapy experience required
• Home health experience required
• Valid driver's license in good standing, daily access to a personal vehicle for business use, and proof of auto insurance required
• Locals accepted — no radius restrictions

Preferred Experience / Skills

• Experience with standardized functional assessments and outcome measurement tools
• Familiarity with electronic health record documentation systems
• Experience serving as a clinical resource or providing in-services to rehabilitation teams
