Physical Therapist (PT)At Parkway Health and Rehabilitation Center, rehabilitation is about more than improving strength or completing exercises — it's about helping residents regain the mobility, confidence, and independence they need to participate in the activities and routines that matter most.We are seeking a compassionate and clinically driven Physical Therapist (PT) to join our rehabilitation team. In this role, you'll provide skilled physical therapy services, develop individualized treatment plans, and work closely with residents to help them achieve meaningful functional goals.Parkway is a 141-bed skilled nursing and rehabilitation facility in Boston, Massachusetts, providing both Short-Term Rehabilitation and Long-Term Care. Our Physical Therapists are more than providers of therapy services — they are partners in helping residents regain mobility, rebuild confidence, maintain independence, and achieve meaningful goals throughout their rehabilitation journey.If you're looking for a rehabilitation environment where your clinical expertise is valued, your contributions are recognized, and your work makes a lasting impact in the lives of others, we invite you to apply today and become part of the Parkway Health and Rehabilitation Center team.ResponsibilitiesAs a Physical Therapist, you will evaluate residents, develop individualized plans of care, and provide skilled physical therapy interventions designed to improve mobility, strength, balance, safety, and functional independence.Physical Therapist Responsibilities:Perform comprehensive physical therapy evaluations and assessments based on resident needsDevelop individualized treatment plans and measurable therapy goals in accordance with physician ordersProvide skilled physical therapy treatments within the scope of practice and established plan of careTreat residents with a variety of orthopedic, neurological, and functional mobility needsHelp residents improve strength, balance, coordination, endurance, mobility, gait, and functional abilitiesImplement therapeutic exercises, functional activities, gait training, balance interventions, and other appropriate treatment approachesMonitor resident response to treatment and modify interventions as clinically appropriateEvaluate resident progress and make recommendations regarding continued therapy, discharge, or maintenance needsProvide education to residents, families, and caregivers regarding exercises, mobility strategies, safety, and discharge recommendationsCollaborate with Physical Therapist Assistants regarding treatment plans, resident progress, and clinical recommendationsParticipate in resident care planning and interdisciplinary team meetingsParticipate in discharge planning and provide recommendations to support successful transitionsMaintain accurate and timely clinical documentation in accordance with state and federal regulationsCommunicate resident progress, changes in condition, and treatment recommendations to appropriate members of the care teamProvide clinical direction and support to Physical Therapist Assistants as appropriateAssist with maintaining a safe, clean, and organized therapy environmentParticipate in continuing education, in-service training, and departmental meetingsMaintain resident confidentiality, privacy, and dignity in all interactionsFollow all safety, infection control, and regulatory standardsPerform other duties as assignedOur Physical Therapists have the opportunity to work with residents experiencing a variety of rehabilitation needs, including:Orthopedic recoveryPost-surgical rehabilitationStroke and neurological recoveryBalance and fall preventionStrength and endurance deficitsMobility and gait impairmentsGeneral geriatric strengthening and mobilityFunctional declineLong-Term Care maintenance therapy needsQuality-of-life and independence-focused goalsQualificationsWhat We're Looking For:Graduation from an accredited Physical Therapy program requiredCurrent Massachusetts license to practice as a Physical Therapist requiredCurrent CPR/BLS certification requiredLong-term care, skilled nursing, or rehabilitation experience preferredExperience working with geriatric and subacute rehabilitation populations preferredStrong clinical reasoning, evaluation, and treatment planning skillsAbility to develop and implement individualized plans of careExcellent communication and interpersonal skillsAbility to collaborate effectively with residents, families, and interdisciplinary healthcare teamsAbility to provide clinical direction and support to Physical Therapist Assistants as appropriateCompassionate, dependable, and resident-focused approach to carePassion for helping residents achieve meaningful functional goalsAbility to meet applicable state and federal regulatory requirementsThis position requires regular interaction with residents, coworkers, visitors, and/or supervisors.
