Full-Time | 37.5 Hours/Week | Community-Based
At Harmonia Collaborative Care, our Health Homes Case Managers help individuals navigate medical, behavioral health, and community services with compassion, consistency, and follow-through.
We're looking for a dependable, organized professional who enjoys helping others and values supportive leadership, manageable caseloads, and work-life balance.
What You'll Do
Provide Health Home care management and coordination services.
Conduct home and community-based visits.
Develop and maintain person-centered care plans.
Connect members with medical, behavioral health, housing, and community resources.
Coordinate services with providers and support systems.
Maintain accurate, timely documentation and meet compliance requirements.
What We're Looking For
Bachelor's degree in human services or related field preferred.
Experience in case management, behavioral health, social services, or community support preferred.
Strong communication, organization, and documentation skills.
Dependable, professional, and compassionate approach.
Computer proficiency, including Microsoft Word and Excel.
Valid New York State driver's license and reliable transportation for home and community visits.
Availability Monday through Friday, starting at 7:30 a.m. (37.5 hours/week).
