About the Role

The Care Manager is responsible for successfully directing and coordinating all activities associated with the care of potential and existing clients. This role provides leadership and management of the overall client experience and coordinates effectively with the office team to ensure that all interactions support the needs and best interests of the client, while supporting the achievement of operational goals.

What You’ll Do

• Build confidence and trust with prospective clients and their families by helping to identify their goals and establish their care needs.

• Develop a formalized and individual care plan which reflects the clients care goals, choices and preferences and provides the Caregivers with all the information required to deliver high-quality care.

• Accurately document all interactions held with client, family, or other stakeholders about the client in the CRM and share relevant information with appropriate team members including feedback about Caregiver Performance.

• Conduct client reviews to ensure client satisfaction with their care services and the highest quality of care is being provided whilst also determining any change in care needs requiring an update of the care plan and change to services.

• Ensure all documentation regarding services are kept up to date.

• Support new Caregiver Onboarding and training.

• Provide on call client/Caregiver support outside of regular business hours as required.

What We’re Looking for

• Licensed Nurse (LPN or RN) is required.

• Valid driver’s license is required.

• Experience working with seniors, preferably experience in identifying, coordinating, and managing care services for Home Care Package clients.

• Experience in identifying appropriate services or resources and building effective relationships to work with multidisciplinary teams across health and community services sectors is preferred.

• Highly efficient and organized with proven time management skills.

• Strong engagement, influencing and negotiating skills.

• Effective communicator with excellent customer service skills and demonstrated problem solving abilities.

• Ability to work both autonomously and as part of a team.

• Self-motivated and outcome orientated, with the capacity to deal with multiple priorities.
