Urrly is seeking a Telehealth Nurse Practitioner to join a remote, member-facing clinical team. You will conduct Transitional Care Management visits within 14 days of discharge, perform comprehensive assessments, reconcile medications, identify safety concerns, and coordinate care to reduce readmissions. You will work remotely across multiple states, documenting encounters same-day and supporting members and caregivers via video visits with HIPAA-compliant practices. #J-18808-Ljbffr Doist
