Nurse Practitioner (NP) Remote Care Model

We're working with a healthcare team focused on managing complex Medicaid populations through a telehealth-first, coordinated care model, designed to improve outcomes while reducing unnecessary hospital utilization.

This role is different because:

• Primarily remote / telehealth-based care model
• Opportunity to help shape a new, value-based care approach
• Work with complex, high-impact patient populations
• Strong interdisciplinary collaboration (without traditional clinic constraints)
• Leadership opportunities available (Lead NP track)

The role includes:

• Lead and coordinate care for complex patient populations
• Practice with a high level of autonomy, including prescribing
• Collaborate with physicians, care managers, and health plans
• Support proactive care management and early intervention
• Assist patients through transitions of care (hospital to home)
• Provide virtual care and guidance to reduce unnecessary ER visits

This role is best for NPs who:

• Have 35+ years of clinical experience
• Are comfortable practicing independently
• Have experience with telehealth or remote patient management
• Enjoy working with complex populations (Medicaid, elderly, behavioral health)
• Thrive in a startup or evolving environment
• Want to have a voice in shaping care delivery models

Schedule & Structure:

• Primarily remote, team-based care model
• Mix of W-2 and 1099 options available

Compensation:

• $130,000 $170,000 (depending on experience and leadership level)

Requirements:

• Nurse Practitioner license (AZ)
• Master's degree or higher
• Strong communication and care coordination skills
