Primary Care Nurse Practitioner (NP) / Physician (MD/DO) – Assisted Living Communities | Full-Time | Albany Region, NY

Location: Hoosick Falls, NY, Cambridge, NY & Troy, NY

Schedule: Full-Time | 4–5 Full Days per Week
• Travel between assisted living communities in Hoosick Falls, Cambridge, and Troy

Compensation:
• Competitive Compensation (Open)
• $5,000 Retention Bonus after 90 Days

Position Overview

We are seeking an experienced Nurse Practitioner or Physician to join a growing healthcare team delivering primary care services to residents in assisted living communities throughout the Albany region. This role offers a rewarding opportunity to provide high-quality, patient-centered care while developing long-term relationships with a geriatric population.

Key Responsibilities

• Deliver comprehensive primary care services to assisted living residents
• Assess, diagnose, treat, and monitor acute and chronic medical conditions
• Develop individualized treatment plans and preventive care strategies
• Collaborate with facility staff, caregivers, specialists, and families
• Ensure accurate documentation and regulatory compliance
• Manage patient care in accordance with Medicare and Medicaid requirements

Required Qualifications

• Active New York State NP or Physician License

• Prior Primary Care experience required

• Currently enrolled and credentialed with Medicare and Medicaid (Mandatory)
• Active DEA Registration
• Nurse Practitioners must have a minimum of 3,600 clinical practice hours and meet NYS independent
practice requirements
