About the Role:
Solara Hospice LLC is seeking a compassionate and dedicated Hospice On Call RN to join our team in Waltham, MA. This is a meaningful opportunity to provide expert clinical care and emotional support to patients and families during life's most important moments. Responsibilities:

• Respond to after-hours patient and family calls, providing clinical guidance and triage support
• Conduct urgent visits to assess and manage acute changes in patient condition
• Administer and adjust medications for pain management and symptom control
• Coordinate with the interdisciplinary hospice team to ensure continuity of care
• Document patient assessments, interventions, and outcomes accurately in the EMR system
• Provide compassionate end-of-life support and education to patients and their families
• Communicate critical updates to attending physicians and on-call medical staff as needed

Requirements:

• Active RN license in the state of Massachusetts
• Minimum 1-2 years of hospice, palliative care, or home health nursing experience preferred
• Strong clinical assessment and critical thinking skills
• Experience with pain and symptom management in end-of-life care
• Proficiency with electronic medical records (EMR) documentation
• Valid driver's license and reliable transportation for home visits
• Excellent communication skills and ability to remain calm under pressure
• Compassionate, patient-centered approach to care
