Physical Therapist Home Health

Overview Position: Home Health Physical Therapist with Ascension at Home, Together with Compassus. The role provides physical therapy services in patients\' homes, delivering care to improve independence and daily living activities. Responsibilities Work one-on-one with patients in their homes to deliver physical therapy interventions. Assess patients\' strength, mobility, balance, and functional goals; develop and implement individualized treatment plans. Collaborate with patients, families, and care teams to support home-based rehabilitation and safe discharge planning. Document patient progress and adjust plans as needed to achieve functional outcomes.
