Our core values promote an environment of respect, courtesy, and professional excellence where you can dedicate yourself to a career you love.JOB RESPONSIBILITIES :Collaborates with dialysis providers to ensure communication between facility and dialysis centers is consistent, accurate, and information is followed uponResponds to resident concerns regarding dialysis careMonitors the process of obtaining accurate weights of dialysis residentsTrains licensed nurses on assessment skills for monitoring dialysis residents and monitors licensed nurse alert charting complianceEnsure staff are competent with dialysis care, vascular access devices, fluid and dietary restrictions, etcCommunicate with medical practitioners and other clinical managers to effectively problem solve and manage the facility dialysis populationParticipate/lead quarterly reviews of resident outcomes, dialysis efficiency, anemia management, and overall dialysis program successQUALIFICATIONS & EXPERIENCE REQUIREMENTS:Registered Nurse in the state of VirginiaRegistered Nurse with at least 5 years medical/surgical experiencePrevious Long Term Care ExperienceTHE COMMUNICARE COMMITMENTFirst and foremost, we are a compassionate, family-owned company who truly believes in the care we provide.
