Nurse Practitioner (NP) – Texas

Location: Alice, San Diego, and Robstown, TX
Job Type: Full-Time

Job Summary:

We are seeking a compassionate and skilled Nurse Practitioner to provide primary and acute care to nursing home residents across multiple locations in Texas. The ideal candidate will focus on proactive patient care, chronic disease management, early identification of health concerns, and reducing unnecessary hospitalizations through timely interventions.

Key Responsibilities:

• Provide primary and acute care to nursing home residents.
• Conduct patient assessments, diagnose conditions, and develop individualized care plans.
• Monitor changes in patients’ health and address concerns promptly.
• Coordinate care with families, nursing staff, and other healthcare providers.
• Educate patients and families about disease management and advance care planning.
• Promote quality care and help prevent avoidable emergency visits and hospitalizations.
  

Qualifications:

• Master of Science in Nursing (MSN).
• Active Texas Nurse Practitioner license.
• Current Nurse Practitioner certification.
• Active DEA prescriptive license.
• Strong clinical assessment, diagnostic, and communication skills.
• Ability to work independently and collaboratively.
• Long-term care experience preferred.
• Bilingual skills are a plus.
