Responsibilities

• Provide skilled nursing care to hospice patients in their homes
• Assess, plan, implement, and evaluate nursing care plans according to patient needs
• Coordinate with interdisciplinary team members to ensure comprehensive care
• Educate patients and families about hospice care and symptom management
• Maintain accurate and timely documentation in compliance with regulatory standards
• Communicate effectively with patients, families, and healthcare providers

Requirements

• Current RN license in the state of Michigan
• Experience in hospice or home health nursing preferred
• Strong clinical assessment and communication skills
• Ability to work independently and as part of a team
• Compassionate, patient-focused approach to care
