Medicaid Care Manager - Adult & Behavioral Health

Alpine Physician Partners is seeking a Care Manager (Social Worker) to deliver clinically and psychosocially informed, person-centered care management to Medicaid members with complex needs. You will conduct comprehensive assessments, manage transitions of care, develop care plans, and coordinate with interdisciplinary teams to improve engagement and outcomes.
