The RN Case Manager facilitates the progression and transition of care using established criteria and in conjunction with the multidisciplinary team. The RN Case Manager will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization. Qualifications Registered Nurse (RN) license 2+ years of experience in case management OR 3+ years of experience in clinical nursing Bachelor’s Degree HCA HealthONE provides a wide range of services across the continuum of care in the Denver metro area, including acute care, pediatric, rehabilitation, urgent care, mental health, imaging, surgery, physician practices, home and hospice care, and regional transport. Presbyterian/St. Luke’s is an acute care hospital with dedicated pediatric services and a strong oncology program.
