$1,500 Sign-on-Bonus! M - F 8am - 4:30pm
The LPN Admissions Care Coordinator primary role will be responsible for reviewing newly admitted hospice patient’s charts for accuracy and will QA all necessary forms needed for admissions. He/she will follow-up with patients on the NTUC list & provide additional information about hospice services.
Responsible for answering incoming telephone calls from patients/caregivers/medical professionals. Ability to react quickly and calmly, exercise good judgement and make sound decisions during urgent and routine communications. Proficient in receiving, obtaining and relaying accurate information to be disseminated. Provides skilled supportive patient and family care over the telephone in accordance to certification restrictions. May be required to visit patients when needed. Provides care within scope of practice.
Qualifications
Current Florida LPN license required.
Current CPR certification.
Minimum of three years of nursing experience in a home health/hospice setting within the past five years is required.
Preferred knowledge of hospice, Medicare, Medicaid and private insurance benefits.
Must be able to handle sensitive issues appropriately using sound judgment, maintain professional boundaries, and maintain confidentiality.
Must be self-directed and able to work independently or as a team.
Basic computer skills, use of common office software and EMR software experience.
Excellent people and relationship-building skills. Excellent customer service skills.
Maintain effective work relationships with co-workers, the public and administration.
Highly organized, adaptable problem solver and ability to work under stress or pressure.
Attendance is a condition of employment. Ability to work a variety of shift hours, to include days, nights, weekends and holidays.

Be current in legal and compliance hospice issues.
Responsible for upholding Treasure Coast Hospice/Model Standards for patient visits and compliant documentation.
Understanding of and ability to execute routine Hospice Nursing: crisis intervention, communication skills, teaching skills, thanatology skills and needs.
Under the supervision of the RN Admissions, when needed, provide skilled professional nursing care to the patient and family unit incorporating physical, psychosocial, spiritual, cultural, and biological components.
Utilize the nursing process, demonstrate accurate data collection, and assist in the planning, implementation and delivery of clinical care services.
Implement the nursing plan of care in accordance to the physician orders and under the RN Case Manager.
Provide appropriate pain and symptom management, including the patient’s response to treatment and medications.
Document patient care which reflects nursing intervention, patient's response to care and progress towards goals as set in the Plan of Care.
Maintain clerical type records via the computer or handwritten data.
Must demonstrate ability to provide an effective, supportive, sensitive approach in response to needs of patients, families and staff. Must be polite and courteous at all times.
Provide program development strategies for Treasure Coast Hospice, in an organized, routine capacity that will enable the cohesion of team developments. Work collaboratively with other departments, demonstrate and encourage leadership for a comprehensive approach to program development. Inspire staff to live the Treasure Coast Hospice’s Mission, to achieve its Vision, and demonstrate our Values and Commitment. Be current in legal and compliance hospice issues.
Teach and uphold Treasure Coast Hospice’s standards with a focus on perfect documentation, perfect visits, perfect communication, and professional dress & behavior.
Lead, develop and motivate employees with confidence, energy, integrity and resources.
Provide verbal assistance over the telephone and care to patients and families.
Participate as an active supporter of the Treasure Coast Hospice compliance program, including an emphasis to report, as well as complying with all Federal, State and Local laws as well as the organization’s compliance program.
Fulfill all responsibilities related to the success of the strategic plan of the organization, as requested and assigned.

Position will be categorized as category II (involves minimal exposure to blood and body fluids/tissues).
