Responsibilities

As an LPN, you'll be an integral part of our clinical team, delivering high-quality care and helping our residents achieve their highest level of wellness and independence.

Your Responsibilities Will Include:

• Administering medications and treatments safely and accurately
• Coordinating resident admissions, discharges, and transfers
• Communicating with physicians, pharmacies, residents, and families
• Monitoring residents' conditions and reporting changes promptly
• Collaborating with the interdisciplinary care team to provide exceptional resident outcomes
• Implementing and evaluating individualized care plans
• Maintaining accurate and timely clinical documentation
• Following infection control, safety, and emergency procedures
• Supervising nursing assistants and support staff as appropriate
• Ensuring resident dignity, privacy, and rights are always protected
• Maintaining knowledge of nursing equipment and proper usage
• Providing compassionate nursing care in a long-term care and rehabilitation setting

Qualifications

• Current Licensed Practical Nurse (LPN) License in the State of New Jersey
• CPR Certification Required
• Strong communication and teamwork skills
• Compassionate, dependable, and resident-focused
