Residential LPN

The incumbent assures optimal medical care for group home residents and assists in establishing sound medical procedures to be implemented by the residential department, with the option for a hybrid work schedule where appropriate tasks can be performed remotely.

Coordinates scheduling and attends medical appointments for residents and provides ongoing communication with physicians regarding resident's health care.

Monitors accuracy of medication administration, comparing physician orders with Medication Administration Record and with prescription labels, including dosages and dates.

Implements physician orders completely and accurately and provides follow-up care and/or observation for residents' illnesses, accidents and seizures.

Initiates, maintains and provides needed follow-up communication with pharmacy to assure physician ordered medications are delivered and available in a timely manner.

Maintains written summaries of nursing contact for each resident, including individual observation and chart review.

Maintains residents' medical files and documentation according to State Board of Health and Medicaid requirements.

Assures electronic medication administration records are updated in real time as medication orders are received.

Administers vaccines/tests as scheduled or as needed, including HEP-B and TB to staff and clients. Schedules and assists in administering quarterly physical and medication reviews.

Provides initial and ongoing health care training of agency personnel pertaining to various medical techniques, including universal precautions, signs and symptoms of illnesses, and seizure disorder management and other medical conditions specific to individuals served. Provide initial and on-going training of state mandated medication administration curriculum.

Responds to medical emergencies and concerns that arise outside of regular working hours.

Participates in each individual's Interdisciplinary team and reports on progress.

Participates on agency committees such as Human Rights Committee, Safety Committee, etc.

Performs related duties as assigned, assuring the best interest of both the agency and the clients' welfare.

Certified as a Licensed Nurse and possession of a current Indiana license in nursing.

Thorough knowledge of and ability to make practical application of PDR, State Board of Health, EDS/Medicaid Review criteria and other applicable regulations regarding health care.

Ability to successfully obtain certification in Core A & B (state-mandated medication administration curriculum) and TB skin test administration within required timeframes and maintain certification thereafter.

Ability to work with minimum supervision, both alone and with others in team environment, often under time pressure.

Ability to work on several tasks at the same time, to read and interpret physician orders and lab reports and to maintain confidentiality of information and reports.

Ability to interact appropriately and effectively communicate orally and in writing with other agency personnel, residents and their families/guardians and other health care providers, including being sensitive to professional ethics, gender, cultural diversities and disabilities.

Ability to conduct physical examinations of residents, including grasping and carrying objects, bending at the waist, crouching and kneeling to provide assistance to residents and/or lifting of residents.

Ability to observe behavior of residents and respond swiftly, rationally and decisively to emergency situations, despite possibility of exposure to potentially violent/irate individuals and situations involving potential physical harm to self and others.

Ability to work irregular and/or extended hours. Responds to medical emergencies and concerns that arise outside of regular working hours.

Ability to follow all personnel policies and rules of the organization.

Possession of a valid driver's license and a demonstrated safe driving record.
