Physical Therapist

As a Physical Therapist, you will be responsible for assessing and treating patients to help them regain physical function and improve mobility. You will work within a collaborative healthcare team and design individualized treatment plans that promote independence and enhance the quality of life for residents.

Key Responsibilities:

• Perform thorough evaluations to assess patients' physical abilities and develop personalized treatment plans.
• Provide physical therapy interventions to improve strength, mobility, and functional abilities.
• Educate patients and their families on therapeutic exercises, safe movement techniques, and home modifications.
• Collaborate with the interdisciplinary team to coordinate care and optimize patient outcomes.
• Monitor patient progress and adjust treatment plans as necessary to ensure the best possible outcomes.
• Maintain accurate and timely documentation of patient assessments, progress notes, and treatment plans in accordance with facility policies.
• Ensure that care plans align with regulatory requirements and best practices in physical therapy.

Qualifications:

• Experience working in a skilled nursing facility or similar healthcare setting is preferred.
• Knowledge of physical therapy techniques, therapeutic modalities, and adaptive equipment is essential.
• Excellent communication, interpersonal, and problem-solving skills are required.
• Ability to work both independently and collaboratively in a team environment is necessary.
• A compassionate, patient-focused approach to care is essential.
