Work Environment

As a Nurse Practitioner at Molino Health Care Clinic, you will primarily be working onsite in patients’ homes, bringing vital medical services directly to those who may have difficulty accessing traditional healthcare facilities. Key Responsibilities

• Conduct thorough assessments and examinations to diagnose both acute and chronic conditions in patients within their home environments.
• Develop, implement, and adjust personalized treatment plans, utilizing evidence-based practice to ensure optimal patient outcomes.
• Order and interpret diagnostic tests such as blood work, imaging, and other relevant evaluations to support clinical decision-making.
• Prescribe medications and administer therapeutic interventions in alignment with treatment goals.
• Educate patients and their families on managing their conditions, including health promotion and disease prevention strategies.
• Maintain detailed, accurate, and timely medical records utilizing electronic health record (EHR) systems to enhance care coordination.
• Collaborate with a multidisciplinary team of healthcare professionals, including physicians, nurses, and social workers to facilitate comprehensive care.
• Participate in quality improvement initiatives to elevate the standard of care provided to our patients.

Qualifications

• Current certification as a Nurse Practitioner by a recognized boards.
• An active and unrestricted nursing license in the state of Georgia.
• A Bachelor’s degree in Nursing; a Master’s degree in Nursing or an equivalent advanced practice nursing degree from an accredited institution is strongly preferred.
• A minimum of three years of clinical experience, specifically in primary care or home health settings, is required.
• Exceptional communication and interpersonal skills, with a focus on patient education and empowerment.

Compensation and Benefits

• Competitive salary ranging from USD 120,000 to 130,000 annually, commensurate with experience and qualifications.
