Work Environment

As a Nurse Practitioner at Sinian Care Center, you will work primarily in patients' homes throughout Columbus, GA, allowing you to provide personalized care tailored to each individual’s needs. This onsite position enables you to foster meaningful relationships with patients, understanding their life circumstances and health challenges in a true home setting. You will collaborate closely with nurses, care coordinators, and other healthcare professionals to streamline care delivery and engage with families effectively.

Key Responsibilities

• Conduct comprehensive assessments and diagnose acute and chronic medical conditions by performing detailed patient evaluations.
• Develop, implement, and evaluate personalized treatment plans that consider patients' specific health concerns and lifestyle factors.
• Order diagnostic tests to monitor patient health and effectively respond to changes in their conditions.
• Prescribe medications and therapies as clinically indicated, ensuring the appropriateness and safety of all treatments.
• Educate patients and their families on disease prevention strategies, management of existing conditions, and overall health promotion.
• Document all interactions accurately in electronic medical records to ensure continuity of care and compliance with healthcare regulations.
• Participate in quality improvement initiatives to enhance service delivery and patient satisfaction.
• Stay informed on current healthcare trends, best practices, and regulatory changes to continually upgrade your professional competencies.

Patient Population / Client Focus

The patients you will serve are predominantly adults with a range of acute and chronic health conditions, including but not limited to hypertension, diabetes, mental health issues, and post-operative recovery needs. Your role will emphasize managing these conditions in a holistic manner, considering social determinants of health that impact your patients’ overall well-being. You will aim to engage not just the patients, but also their families, in developing comprehensive care plans that promote better health outcomes.

Qualifications

• Current Nurse Practitioner certification from an accredited institution.
• Active nursing license in the state of Georgia.
• Bachelor’s degree in Nursing; a Master’s degree or higher is preferred.
• A minimum of 3 years of clinical experience in a direct patient care role, with home health experience being a strong advantage.
• Exceptional communication and interpersonal skills to interact with patients and collaborate effectively with the healthcare team.
• Strong analytical and critical thinking skills to assess complex situations and develop effective solutions.

Preferred Experience

• Demonstrated experience in home care or community health nursing.

Compensation and Benefits
