Are you passionate about caring for seniors and making a meaningful impact on their quality of life? We're looking for an experienced Nurse Practitioner to join our team and provide exceptional care to residents in an Assisted Living Facility (ALF). In this role, you'll partner with residents, families, and interdisciplinary care teams to deliver compassionate, patient-centered care while helping residents live healthier, fuller lives.

What You'll Do

• Provide comprehensive primary care to elderly and medically complex residents
• Conduct assessments, manage chronic conditions, and develop individualized care plans
• Collaborate with families, caregivers, facility staff, PCPs, and specialists
• Lead advance care planning discussions and complete POLST forms
• Perform routine resident rounds and respond to clinical needs
• Review diagnostic results, prescribe treatments, and manage medications
• Maintain timely and accurate EMR documentation
• Participate in interdisciplinary team meetings and quality improvement initiatives

What You Bring

Required

• Active Nurse Practitioner license
• DEA registration
• Minimum 3 years of geriatric care experience
• Recent Assisted Living Facility (ALF) experience
• Strong clinical judgment and communication skills
• Ability to work independently in a fast-paced environment

Preferred

• Adult/Geriatric Nurse Practitioner certification
• Long-term care or skilled nursing experience
